ABSTRACT

Glaucoma is one of the most common optic neuropathies, featuring progressive retinal ganglion cell damage and visual field loss (Tham et al., 2014; Xu et al., 2020). Currently, the only effective treatment for this condition is the reduction of intraocular pressure (IOP) (Palmberg, 2001; Heijl et al., 2002). Canaloplasty is a proven bleb-independent surgery with good efficacy and safety profiles in primary open-angle glaucoma (POAG) (Gołaszewska et al., 2021). However, early transient postoperative IOP elevation has been reported in up to 30% of cases (Riva et al., 2019), similar to that commonly observed in other internal drainage glaucoma surgeries such as implantation using iStent (0%-21.0%), CyPass (10.8%), and Hydrus (4.8%-6.5%) (Lavia et al., 2017). This complication may be a predictor of poor reserve in the outflow system and is potentially associated with surgical failure. Nonetheless, the exact pathophysiology of glaucoma remains unknown, and studies clarifying the risk factors for postoperative IOP elevation have been scarce.

ABSTRACT

Creeping hair is a rare condition characterized by creeping eruption with a black line at the advancing end, mimicking cutaneous larva migrans. The condition is also referred to as cutaneous pili migrans, migrating hair, and embedded hair. A total of 52 cases have been reported since 1957 and most cases were published in English. Herein, we report a case in which creeping hair occurred in the iliac region and review the literature from 1957 to February 2021. A 35-year-old Chinese female presented with a black moving linear eruption that had migrated from the lower abdomen to the iliac region without causing any symptoms during a 3-year period. Cutaneous examination showed a 6.5-cm-long black linear lesion beneath the skin that was revealed to be a hair shaft. After removal of the hair, the eruption diminished and no recurrence occurred in 3 months of follow-up. The creeping hair that had migrated with its lower end forward was confirmed by observation under dematoscopy and light microscopy. A review of the literature revealed that creeping hair occurs most frequently in young and middle-aged patients and the reported cases are mainly from Asia. The top locations involve the foot. The causative hair includes head hair, beard, pubic hair, body hair, and one case of dog hair. A closeup examination and dermoscopic inspection are helpful for the diagnosis of creeping hair.

ABSTRACT

【Objective】 To explore the challenging blood cross-matching and resolution for multiple myeloma (MM) patients in different disease stages. 【Methods】 For a patient who was first diagnosed as MM and scheduled for blood transfusion, his blood was cross matched with donors’ blood by microcolumn gel method and tube test. When the major side of cross-matching was agglutinated, the patient’s plasma was cross matched with donors’ red blood cell (RBC) by polybrene test, then plasma dilution cross matched with donors’ RBC by microcolumn gel method. For a patient who was diagnosed as recurrent refractory MM and scheduled for blood transfusion, his blood was cross matched with donors’ blood by microcolumn gel method. 【Results】 1) Case 1 was a first-visit outpatient. The major side of microcolumn cross-match test was agglutinated with the shape of fine line. The result of tube method also showed agglutination of major sides, and the rouleaux were detected by the microscopy. Then polybrene method and microcolumn gel method (after plasma diluted) were applied for cross-matching again with the above two donors’ blood and showed compatibility. 2) Case 2 was a recurrent refractory MM patient. The major and minor sides of microcolumn cross-match test were both agglutinated with the shape of granular. The patient was treated with anti-CD38 monoclonal antibody. The RBCs, after treated with dithiothreitol (DTT) was used to cross match with patient plasma by microcolumn test, and the result was compatible. 【Conclusion】 Polybrene method and microcolumn gel method after plasma diluted are suitable for blood cross-matching of newly diagnosed MM patients, also for those treated with CD38 monoclonal antibody, as the drug interference with cross-matching can be eliminated by DTT.

Objective:To understand the screening of cervical cancer in our county and provide epidemiological data for the prevention and control of cervical cancer. Methods:A total of 3 034 healthy women with 30~65 years old in Chun'an county were recruited in this prospective study from June 2015 to January 2016.All of the subjects received cervical cancer screening. The respondents were randomly divided into non-typeable HPV screening group (2018 cases) and the vision inspection with acetic acid and visual inspection with Lugol's iodine screen (VIA/VILI) group (1016 cases). Direct colposcopy or VIA/VILI visual examination was performed for those with positive HPV, and then colposcopy was performed for those with positive visual examination. The suspicious lesions tissues were applied for pathology detection. From September 2017 to February 2018, 2 363 patients who were initially negative received VIA/VILI combined with unclassified HPV examination, and those who were positive received colposcopy and pathological biopsy. The positive rate of primary screening and cervical squamous intraepithelial neoplasia disorders level 2 and above (CIN Ⅱ + ) lesion detection rate were analyzed in the two-stage screening. Results:From June 2015 to January 2016, the preliminary screening positive rate of unclassified HPV group was 11.60% (234/2 018). A total of 150 patients with positive results were recalled for colposcopy, with a recall rate of 64.10% (150/234). Among the 150 cases, 119 cases (79.33%) were positive in the unclassified HPV preliminary screening, and 31 cases (20.66%) were positive in VIA/VILI examination after HPV preliminary screening. The positive rate of VIA/VILI initial screening group was 20.28% (206/1 016), and the recall rate of colposcopy was 99.51% (205/206). A total of 108 cases underwent colposcopy with abnormal pathological biopsy examination in the two groups, and the pathological biopsy rate was 30.34% (108/356), and the pathologic biopsy CIN Ⅱ+ lesion detection rate was 297/100 000 (9/3 034). In the second round of screening from September 2017 to February 2018, 2 363 subjects were recruited back to the baseline, with a recall rate of 77.89% (2 363/3 034). In the 2 363 cases, the early screen VIA/VILI anomalies or HPV positive early screening positive rate was 17.01% (402/2 363), and the colposcopy examination the recall rate was 80.10% (322/402), and 80 cases received vaginal lens suspicious pathological biopsy, the pathological biopsy rate was 24.84% (80/322), the pathological results showed that CIN Ⅱ+ detection rate of 296/100 000 (7/2 363).Conclusion:The detection rate of precancerous suspicious lesions in cervical cancer screening in our county is lower than the national average level. On the one hand, we should strengthen the propaganda and education of active screening, improve the compliance and cooperation degree of screening objects, and improve the recall rate of suspicious cases. On the other hand, the government should invest more efforts to select the screening technology with high sensitivity, improve the screening technology of doctors at the basic screening point, and improve the detection rate of cervical cancer prelesions.

ABSTRACT

An important and unresolved question is how human brain regions process information and interact with each other in intertemporal choice related to gains and losses. Using psychophysiological interaction and dynamic causal modeling analyses, we investigated the functional interactions between regions involved in the decision-making process while participants performed temporal discounting tasks in both the gains and losses domains. We found two distinct intrinsic valuation systems underlying temporal discounting in the gains and losses domains: gains were specifically evaluated in the medial regions, including the medial prefrontal and orbitofrontal cortices, and losses were evaluated in the lateral dorsolateral prefrontal cortex. In addition, immediate reward or punishment was found to modulate the functional interactions between the dorsolateral prefrontal cortex and distinct regions in both the gains and losses domains: in the gains domain, the mesolimbic regions; in the losses domain, the medial prefrontal cortex, anterior cingulate cortex, and insula. These findings suggest that intertemporal choice of gains and losses might involve distinct valuation systems, and more importantly, separate neural interactions may implement the intertemporal choices of gains and losses. These findings may provide a new biological perspective for understanding the neural mechanisms underlying intertemporal choice of gains and losses.

ABSTRACT

Emerging evidence has indicated that BRCC 36-mediated K63-linked ubiquitination modification was involved in diverse cellular functions , including endocytosis , apoptosis and DNA damage repair .We previously showed that activation of cGMP/PKG pathway con-tributed to the binding of BRCC36 and the pro-fibrotic factor Smad3.The current study tested the hypothesis that BRCC 36 functions as a negative regulator of transforming growth factor-beta ( TGF-β)/Smad3 pathway and participates in cardiac remodeling .In isolated adult mouse cardiac fibroblasts , we have demonstrated that TGF-β1 treatment significantly increased the expression of BRCC 36.Over-expression BRCC36 suppressed TGF-β1-induced Smad3 phosphorylation, nuclear translocation, extracellular matrix molecular expres-sion and cell proliferation .On the contrary, silencing BRCC36 by transfection of adenovirus-carrying BRCC36 shRNA potentiated to enhance the pro-fibrotic effect of TGF-β.In vivo, under chronic pressure overload condition-induced by transverse aortic constriction , myocardial pro-survival protein Bcl-2 and Mcl-1 expression were significantly decreased and the pro-apoptosis protein Puma was in-creased.However, the cardiac-specific over-expression of BRCC36 significantly increased myocardial Bcl-2 and Mcl-1 and inhibited Puma expression .Interestingly , we also found that sustained pressure overload resulted in a significant myocardial DNA injury in wild type mice, which was characterized by the increase of γH2AX level. However, cardiac-specific BRCC36 over-expression significantly decreased the level of γH2AX in the pressure overloaded heart in the transgenic mice , while effectively enhanced myocardial RAD 51 expression, a marker of DNA damage repair.Furthermore, BRCC36 over-expression effectively attenuated TAC-induced cardiac fibro-sis and remodeling in the transgenic mice , compared with the wild type mice .Collectively , the results have suggested that BRCC 36 ef-fectively protected heart against chronic pressure overload-induced cardiac remodeling though antagonizing TGF-β/Smad3 pathway and enhancing myocardial DNA injury repair response .

ABSTRACT

Salmonella enterica serovar Choleraesuis strain C500 is a live attenuated vaccine that has been widely used in Chi‐na for over 50 years to prevent piglet paratyphoid .However ,as C500 is obtained by chemical methods ,the genetic background of this strain remained unclear .In this study ,we compared the genomic differences between the virulent reference strain C 78‐2 and C500 by suppression subtractive hybridization combined with the mirror orientation selection method (MOS‐SSH ) .Six genes (asr ,ydgF ,ydgD ,ydgE ,rpoS ,and ptsG) were lost in C500 strain .Using real‐time PCR analysis ,we demonstrated that the genes regulated by rpoS ,a vital transcriptional regulator playing an important role in Salmonella infection ,were downregulated in C500 .Additionally ,the virulence of the rpoS mutant strain C78‐2ΔrpoS was 100 000 times lower than the parental strain in BALB/c mice .So loss of rpoS gene is the major factor leading to the attenuation of C500 strain .

Objective To study the nutritional risks in elderly patients with prostate cancer and explore its related factors. Methods 346 elderly patients with prostate cancer in Wuhan area from May 2013 to May 2014 were chosen as the objects in this study.Nutritional risk screening 2002 was used to evaluate nutritional risk. The sleep quality, anxiety, lower urinary tract symptoms, differentiation and other data of patients were collected. The influencing factors for nutritional risk in the patients were analyzed. Results The ratio of nutritional risk in elderly advanced prostate cancer patients was 35.3% (122/346).The nutritional risk was increased with age (P<0.05).The prostate cancer patients with nutritional risk had a higher international prostate symptom score (IPSS) (P< 0.05).Monovariate factor analysis showed that sleep quality, operation, pathology staging, radiotherapy, chemotherapy, lower urinary tract symptoms (LUTS) were the impact factors for nutritional status in elderly patients with prostate cancer (P<0.05).Logistic regression analysis showed that age (OR=0.29),sleep quality (OR=0.25) were the protective factors for the nutritional status, while surgery (OR=12.67),pathological staging (OR=1.65),radiotherapy (OR=2.65),SPSS (OR=1.55),chemotherapy (OR=1.85) were the risk factors for nutritional status (P< 0.05).Conclusions The incidence of nutritional risk is high in elderly prostate cancer patients.Age,sleep quality are the protective factors, and operation, pathology staging,SPSS,chemotherapy, and radiotherapy are the risk factors for nutritional status.

ABSTRACT

We report 3 cases of successful pregnancies in women with a history of surgeries for gynecological malignancies and postoperative infertility, achieved by in vitro fertilization-embryo transfer (IVF-ET) with controlled ovarian hyperstimulation. All the 3 patients had clinical pregnancies without cancer recurrence. In such cancer survivors with infertility, the ovarian reserve is severely impaired by cancer therapies and assisted reproductive techniques should be the primary option.
